Sponsorship Form

• Tax-deductible receipts will only be issued to those upon request with donations of HK$100 or above.
• Participants can fill in the Sponsorship Form below with sponsor's name and sponsorship amount. Please check in the “Receipts” column if tax-deductible receipts are needed.
• The names provided in the “Sponsor’s Name” column will be printed on the receipts.
• If the sponsor is a corporation / organisation, please write the name of the corporation / organisation for the “Sponsor’s Name”
• Participant can submit the Form after successfully registered for and donate to the event. The deadline for the Form submission is 29 February, 2024.
• Participants can donate extra sponsorship by bank transfer or post the donation cheque to us before 29 February, 2024.
• E-receipts will be emailed to participants on or before 15 March 2024.

You can deposit the donation directly into our account. After completing the transaction, kindly email us the transfer slip or the screenshot of the online transaction record for our record. Please state your name and participant number in the email.

You can send the donation cheque to our office. At the back of the cheque, please write down your name participant number and contact number for our reference.
